The information given on this form is true to the best of my knowledge. It is understood that any false or misleading information may lead to a prosecution for criminal offences and may result in eviction from any housing offered.

Any information given by me/us relating to this housing application, or given with my/our consent by others, will be placed on the HomeChoiceBristol register, I/we understand that any landlord or supported housing provider who takes part in the register, either now or in the future, may see it.

I/we give permission for you to contact individuals or agencies referred to by me/us on this form in order to seek additional information about specific housing needs.

The information you provide will be recorded on a computer system and is covered by the provisions of the Data Protection Act. You are entitled to see the information you have provided.

Bristol City Council has a duty to protect the public funds it administers and may use the information you have provided to obtain housing with this authority for the prevention and detection of fraud. It may also share this with other bodies administering public funds solely for these purposes.

YOU WILL NEED TO PROVIDE TWO FORMS OF IDENTIFICATION FOR EACH ADULT ON THE APPLICATION, one of which must show proof of address, i.e a recent utility bill, and another should show date of birth. You will also need to provide proof of all children listed on your application i.e a child benefit award letter. Please take these documents into your nearest Customer Service Point.
